MaCES students, if you need a work permit please follow the following instructions:

1) Print the PDF form titled Work permit request.

2) Fill out the part where it says Minor's Information. 

3) Fill out the part that says To be filled in and signed by parent or legal guardian.

​If I don't have a parent signature I cannot issue your permit. 

4) Completely fill out the part that says To be filled in and signed by employer. I need this information to be able to issue your work permit. 

I will take care of all the school information.

Once you have a job offer and you have completed the form following steps 1-4, email Ms. Ostos at aostos@lausd.net or Mr. Velasco at dxv0000@lausd.net to schedule an appointment to sign your Work Permit. They will be on campus during summer school hours but you need to make an appointment so they can get back to you. Any questions, please call the office (323)210-1412.
